12/6/2023 EnchantaVaughn Approved QMHPA arrived to the home of Chevelle. Upon arrival to the home, Chevelle appeared to be in a relaxed mood evident by her facial expressions. QMHPA inquired about her day. Chevelle verbalized to QMHPA that her day was going okay. QMHPA engaged Chevelle in a discussion to determine why she appeared to be stressed. Chevelle stated to QMHPA that she was stressed due to her not getting enough sleep again on the previous night. QMHPA engaged Chevelle in a discussion to determine why she wasn’t getting enough sleep. She stated to QMHPA that she didn’t want to talk to QMHPA as to why she wasn’t getting the sleep that she needed. QMHPA informed Chevelle that this session was going to continue to focus on her ISP goals Chevelle did not present any symptoms of SI or HI. QMHPA began the session by inquiring about Chevelle current thoughts, feelings and emotions. QMHPA allowed Chevelle to discuss with QMHPA about her moods and behaviors since the last session. QMHPA inquired specifically about Chevelle depressed moods and techniques she has used to cope with those moods if any. QMHPA reviewed depression coping skills with Chevelle and asked the Chevelle to identify which skills she usually uses to cope with her depression. QMHPA engaged Chevelle about a discussion surrounding current suicidal ideations. QMHPA informed Chevelle that QMHPA was going to assist her with scheduling updated psychiatric appointments to ensure mental health stability. Chevelle appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Chevelle stated she has not been depressed today but she don’t know how long that is going to last. Chevelle stated she typically likes to talk to her friend, watch television or go out with her cousin as techniques to cope with her depression. She stated she does not like to be in the house or bored because that is when she is more likely to become depressed.
